Medvedev mounted a stunning comeback from 6-3, 3-2 down with Djokovic 40/30 up to go 4-2 up on the Russian and likely claim the match from there.
He denied the break point and flipped the match on it's head prevailing 3-6, 6-3, 6-3 in one hour and 43 minutes and marks the first time since 2013 that Djokovic has lost an ATP Masters 1000 semi-final when winning the first set.
Medvedev rushed his serve for the most part to try and get the better of the defending champion and it worked with him hitting 16 aces, the most Djokovic has allowed in a three set tie this year.
He will now face off against Goffin in an unexpected end to proceedings in Cincinnati but one that based on form should be a brilliant one.
Djokovic lauded praise on his opponent after losing out and he called him one of the best players in the world and one that keeps improving whenever he faces him.
"He's working his way to the Top 5 [in the ATP Ranking]. He's definitely one of the best players in the world... He certainly improved since Australian Open this year. His forehand is better. His movement is better. I mean, his serve was always big," said Djokovic.
'I did not experience this too many times in my career that someone goes so big on second serves and just serves two first serves basically for an extended period of time.'
"That's one of those things where you just take your chances. I don't think I can kind of say that he improved his second serve. He was serving two first serves from one point of the match til the end of the match.
"But before that, when I could take my chance on the second serve, I was doing pretty well. But there is certainly an evolution in his game and improvement. He's going in the right direction and it's why he's doing so well."
